The RCP will go to your Club President. You must follow up to make sure your Club President receives this RCP prior to their club meeting for approval. All RCP's from the club must be signed by Club President and mailed to your Regional Director, postmarked no later than 9/15/2012.
All submissions must be typed (you can use the downloadable form then print it out). Submissions can also be e-mailed directly to your club president if their address is in the database. If your club President does not have an e-mail address, you must print this form out after completion and send it by mail or hand deliver. Please fill out all spaces provided on the form.
